My memory book, April 19, 2012 By A. I have almost filled my second book. It fits in my purse so can go everywhere with me. I have had a long 3 years of medical issues, i pull this notebook out at EVERY appointment, make note of pulse, BP. I note what the doctor tells me and what he plans. I can look back and tell him what other doctors have said. I also "diaried" how i feel, symptoms, aches, pains; Exercise. These are thing that i need to remember for doctors and for myself. The book is with me, i whip it out and make a note.
I aboslutely couldn't have come through these years with out it.
Because it is with me always, it also serves as a trip diary; phone number, address book. Holds info til I can move it into more permanent records.
